The DOE has developed visualized information charts for its COVID-19 quarantine policy. These charts have identical information to the DOE's "Health Screening Questionnaire" (https://healthscreening.schools.nyc). The latest version is available on their website at https://www.schools.nyc.gov/school-life/health-and-wellness/covid-information/health-and-safety-in-our-schools (see under "COVID-19 Guidance Charts").
